Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients according to an individualized plan of care and help them achieve measurable functional goals.

You will collaborate with facility staff, provide excellent patient and family service, supervise Physical Therapist Assistants, re-evaluate progress, and complete timely documentation. A sign-on bonus is available.

Qualifications

• Licensed Physical Therapist in Illinois.
• Experience evaluating and treating patients in SNF settings.
• Ability to set measurable, functional goals and document progress.

Responsibilities

• Provide excellent patient care.

• Collaborate with facility staff to ensure high-quality care for patients.

As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
