Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure high quality outcomes.

Join our dynamic team and provide excellent patient care while supervising Physical Therapist Assistants and maintaining thorough documentation. A sign-on bonus is offered.

Qualifications

• Licensed Physical Therapist with NY license.
• Experience in skilled nursing facility (SNF) settings preferred.
• Strong communication and documentation skills.

Responsibilities

• Providing excellent care for our patients.
• Providing excellent customer service to patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, educating them on injuries, progress and probable outcomes.
• Supervising Physical Therapist Assistants.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
