• Evaluate and treat patients following an individualized plan of care in an SNF setting.
• Collaborate with facility staff to ensure high-quality patient care and service.
• Evaluate new patients, develop plans of care and set measurable, functional goals.
• Treat patients, answer questions and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants to support patient progress.
• Re-evaluate patient progress and revise plans of care accordingly.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re‑evaluating patient progress, revising plans of care and resetting 
• Complete timely and accurate documentation including; evaluations, re‑evaluations , discharges and daily progress notes.
