Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care while delivering excellent care and customer service to patients and families.

In this role you will collaborate with facility staff, supervise PTAs, evaluate new patients, set functional goals, and complete timely documentation. This position offers a $2,000 sign-on bonus and generous online CEUs.

Responsibilities

• Collaborate with facility staff to ensure high quality care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
