Job summary

Evolve Therapy Services, LLC is seeking a Part-Time Physical Therapist for an in-house Skilled Nursing Facility in Clearfield, PA. This role includes PRN hours and requires license activation in the relevant state, with responsibilities spanning evaluation, treatment planning, and interdisciplinary collaboration.

The PT will deliver therapy services, document according to CMS and company procedures, and participate in ongoing professional development.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents to determine treatment plans and goals.
• Develop short- and long-term treatment plans and communicate results to residents and families.
• Conduct screenings to determine need for intervention and monitor progress.
• Complete CMS-compliant, medically necessary documentation in NetHealth EMR.
