Job summary

Evolve Therapy Services is seeking a licensed Physical Therapist to provide in-house SNF services in Erie, PA. The role offers Full Time, Part Time, or PRN positions and requires compliance with federal, state, and local standards, with emphasis on resident quality of care.

You will plan, implement, and evaluate therapy programs, perform screenings, and ensure accurate documentation in the CMS-compliant NetHealth EMR system. Opportunities for growth exist within the organization.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up.
• Conduct regular screenings of residents to determine intervention needs.
• Develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
