The role emphasizes counseling, advance care planning, care coordination and resource support, with a focus on patient- and family-centered planning and discharge assistance.

Qualifications

• Master’s degree in Social Work accredited by CSWE.
• LCSW-C licensure from the Maryland Board of Social Work Examiners.
• Minimum three (3) years of post-Master’s experience is required.

Responsibilities

• Completes specialized assessment(s) in a focus area; may advise other social workers.
• Provides psychosocial guidance and advances care planning discussions with the team.
• Delivers advanced ongoing and crisis care within a multidisciplinary setting.
• Leads complex patient cases and networks with program resources.
• Participates in unit practice conversations and quality improvement efforts.

Education

Master’s degree in Social Work (CSWE accredited)

LCSW-C licensure
