Job summary

Qualifications

• Master's degree in Social Work (MSW) required or equivalent.
• At least 1 year of social work experience in acute-care settings.
• Maryland LGSW/LCSW/LCSW-C licensure required.
• Ability to work with patients of all ages.
• Effective communication and time management skills.
• Reports to Director of Case Management/Manager.

Responsibilities

• Collaborate with Care Coordination and multidisciplinary team on discharge planning.
• Arrange for home or alternate setting care as needed.
• Provide counseling to patients/families for adjustment, bereavement, and illness changes.
• Document interventions and outcomes in the electronic record.
• Participate in multidisciplinary rounds with relevant teams.
• Prioritize tasks and work independently or as part of a team.
