Job summary

UP Health System Hospice is hiring a dedicated Social Worker to support patients and families within a multidisciplinary team, under physician supervision, delivering compassionate medical social services.

The role emphasizes evaluating social and emotional factors affecting health, coordinating a comprehensive Plan of Care, and guiding patients toward community resources; 20–25 hours weekly. Benefits include a comprehensive package, incentive programs, 401k and stock options where applicable.

Qualifications

• Master's Degree from an accredited School of Social Work.
• Current Driver's License and access to reliable transportation.
• 1+ years of social work experience in a health care setting.
• Licensed Social Worker in the state of residence.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Assists patient, family, physician and health care team to understand social factors affecting health status.
• Contributes to development of a comprehensive, integrated Plan of Care daily.
• Instructs team on community resources available for patients as needed.
