MedStar Health is seeking a Social Worker to provide psychosocial assessment, counseling, and care coordination for patients with serious illness and their families. Qualifications include a CSWE-accredited MSW, active LGSW/LMSW in MD or DC, and LCSW-C licensure; palliative care experience is preferred.

Qualifikationen

• Master’s degree in social work from a CSWE-accredited program.
• Active LGSW/LMSW license in MD or DC at hire; must obtain licensure in the second jurisdiction within 90 days.
• LCSW-C licensure is required.
• Palliative care or hospice social work experience preferred.

Aufgaben

• Collaborates with interdisciplinary care teams to assess psychosocial needs and develop care plans.
• Provides counseling and emotional support for illness adjustment, stress, grief, loss, and caregiving.
• Leads goals-of-care discussions and advance care planning in family meetings and rounds.
• Coordinates transitions, referrals, and community resources while advocating for equitable access.
• Documents assessments in the EHR and participates in supervision toward licensure.
