Qualifications

• Master's degree in Social Work from an accredited program.
• LCSW licensure required; CHP-SW certification within 2 years.
• Experience in palliative care, end-of-life, or hospice settings.
• Proficient with Windows-based programs and email/Word applications.

Responsabilités

• Performs and documents psychosocial assessments.
• Facilitates patient-family meetings for goals and planning.
• Provides emotional support for patients and families.
• Coordinates educational and outreach services related to social care.
• Participates in palliative care quality improvements.
• Liaises with outpatient/inpatient teams, discharge planning, and social services.
• Links with community hospice, home care, and long-term care services.
• Attends monthly team meetings for operations and administration.
