Job summary

Mary Washington Healthcare is seeking a Palliative Care Social Worker (Outpatient) to conduct psychosocial assessments and provide social work services for patients and families within an interdisciplinary care model focused on comfort and quality of life.

The role requires a Master’s in Social Work, LCSW license, CHP-SW within 2 years, and BLS CPR at hire. Proficiency with Windows-based programs, email, and Word is preferred, with strong teamwork and communication abilities.

Qualifications

• Master's degree in Social Work from an accredited program.
• LCSW licensure required.
• CHP-SW certification within 2 years of hire.
• BLS CPR required at hire.
• NPI required for outpatient billing.
• Experience in palliative care, hospice, end-of-life care.
• Proficiency with Windows, email, and Word.

Responsibilities

• Perform psychosocial assessments and document findings.
• Facilitate patient-family meetings for goals of care and advance care planning.
• Provide emotional support to patients and families facing advanced illness.
• Coordinate educational and outreach services for the social work/palliative care interface.
• Participate in quality improvement initiatives for palliative care.
• Liaise with outpatient and inpatient teams, discharge planning, and community services.
• Attend team meetings and perform other duties as assigned.
