AlignMed Partners is seeking a part-time Palliative Care Provider (24 hours per week) for a float position across three centers in Phillipsburg, NJ: Lopatcong Center, Complete Care at Phillipsburg, and Complete Care at Brakeley Park. This role can be filled by a Nurse Practitioner or Physician Assistant and requires at least 1 year of palliative care experience.

Base pay is $58-$61 per hour plus our incentive program.

Competenze

• The Advanced Practice Provider must be a graduate from an accredited School of Nursing and licensed as an RN.
• Certified as a Nurse Practitioner with a MSN and relevant concentration.
• APRN license with DEA for states practiced.

Mansioni

• Provide primary and urgent care to nursing center residents in collaboration with the medical team.
• Focus on pain and symptom management, psychosocial support, and advance care planning.
• Educate and support patient families; participate in quality initiatives.
