Samaritan Healthcare & Hospice in Mt Laurel, NJ is seeking a diligent Nurse Practitioner for our Palliative Medicine Outpatient team. The NP will provide consultations and follow-up visits across outpatient, home, and clinic settings, collaborating with primary and specialty physicians to deliver patient- and family-centered care.

Qualifications

• NJ RN and APN licenses required or in process.
• DEA and CDS licenses are required.
• Palliative Certification within two years of hire.
• Experience in palliative care, oncology, heart failure, geriatric care, or hospice preferred.

Responsibilities

• Provide health assessments and symptomatic management.
• Collaborate with patients, families, and physicians for goals of care.
• Develop relationships with internal and external partners.
• Coordinate care across Samaritan programs.
