Job summary

AlignMed Partners is seeking a full-time Palliative Care Nurse Practitioner for a float position among our Pittsburgh-area centers, with no on-call or weekend requirements. The base salary ranges from $114,000 to $120,000 per year, plus a no-cap incentive compensation program.

The Advanced Practice Provider will deliver primary and palliative care to nursing center residents, collaborate with the care team, and support patient- and family-centered goals and education.

Qualifications

• Graduate from an accredited School of Nursing, licensed as a registered nurse, certified as a Nurse Practitioner with an MSN and an Older Adult and/or Family NP concentration
• Minimum two years experience with adult/geriatric population preferred
• APRN with unrestricted professional license and DEA for each state of practice

Responsibilities

• Provide primary and urgent care to nursing center residents in collaboration with other medical providers
• Deliver palliative care services focusing on pain and symptom management, psychosocial support, and advance care planning
• Assess health status, interpret labs, develop and implement treatment plans in collaboration with the care team
• Educate patients and families and contribute to clinical education of nursing staff
• Participate in center-based quality initiatives and efforts to avoid hospital transfers
• Adhere to coding, billing compliance and AlignMed Partners policies
