Samaritan Healthcare & Hospice in Mt Laurel, NJ is seeking a part-time Nurse Practitioner to provide palliative care in outpatient and home-based settings. The NP will assess, diagnose, and manage symptoms, collaborating with patients, families and physicians, while building relationships with internal and external partners.

The role is per diem with afterhours limited call rotation, and requires NJ RN/APN licenses, DEA and CDS licenses, and palliative certification within two years.

Qualifications

• Experience preferred in palliative care, oncology, heart failure, geriatrics, or hospice.
• RN, APN license in NJ.
• DEA and CDS licenses required.
• Palliative Certification within two years of hire.

Responsibilities

• Provides health assessments and diagnoses; manages symptoms and goals of care.
• Collaborates with patients, families and physicians for care.
• Develops and maintains internal/external relationships with team and partners.
• Coordinates care across Samaritan programs for all patients.
• Per diem role with afterhours limited call (telephone only).
