For this position, Physician Assistants will manage patients in an outpatient clinic setting as well as providing in-home clinical support for patients in the community. GGH PA's will be supporting clients with their patient panels and partnering with clinics as an embedded member of the care team with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
