Job summary

Heartshare is seeking a Part-Time Direct Support Professional to provide person-centered services in Brooklyn, NY. You will implement individualized plans, support daily living skills, and help individuals participate in community activities.

The role requires a High School Diploma or GED; NYS driver's license; overnight shifts with up to 25% travel. Compensation starts at $19.00 per hour with potential increases and a discretionary bonus.

Qualifications

• High School Diploma, GED or CNA strongly preferred but not required.
• NYS driver's license required.
• Experience with individuals with developmental disabilities preferred.

Responsibilities

• Implement individualized plans and participate in planning activities to address goals.
• Support individuals in skills development, socialization, and task participation.
• Promote community inclusion and assist with transportation and outings.
• Maintain accurate documentation and ensure confidentiality and safety.
• Provide supervision and ensure staff are trained on the LOS as needed.
