Qualifications

• Requires Occupational Therapy evaluation and treatment planning
• Involves patient and caregiver education on evaluation findings and care management
• Collaborates with other disciplines to arrange follow-up care and return to home/work environments

Responsibilities

• Conducts occupational therapy evaluation procedures according to professional and departmental guidelines.
• Responds to consults within 24 hours; completes evaluation and documentation within 48 hours.
• Reviews patient records and obtains physician order prior to assessment.
• Interprets evaluation results and develops diagnostic impressions with recommendations.
• Establishes treatment goals and plans that meet patient needs and reimbursement requirements.
• Educates patient and caregiver about evaluation findings and management recommendations.
• Assists in discharge planning and coordinates follow-up as needed.
