Nurse Practitioner, Advanced Practice Provider
Provide effective, equitable primary care to adult Medicare patients, including routine and acute visits, wellness exams, care coordination, and patient education. Practice independently while collaborating with physicians and a multidisciplinary team. Lead population health initiatives, assist with phone triage and outreach, conduct home visits if needed, and participate in promotional activities. Work closely with operational leadership and support a network of specialists and hospitals.
Master's or Doctorate in Nursing, with active certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner, and valid state license with DEA registration. Must have US work authorization. Preferred experience in primary care, understanding of HCC and ICD-10 coding, and bilingual skills are advantageous.
