Job summary

The VIOS Clinic is seeking a Nurse Practitioner to deliver comprehensive outpatient care to adult Medicare patients in a collaborative team setting. In this role you will conduct visits for wellness and acute issues, coordinate with specialists, and contribute to population health initiatives.

Ideal candidates hold an MSN, DNP or NP certificate, with national certification and an active NP license (DEA where required) and US work authorization.

Role:

Nurse Practitioner, Advanced Practice Provider

Responsibilities

• Provide high-quality, equitable primary care to adult Medicare patients, fostering trust and meaningful relationships.
• Conduct office visits for routine and acute issues, including annual wellness visits and health risk assessments.
• Coordinate care with specialists, testing facilities, and other providers to ensure comprehensive patient management.
• Lead population health initiatives, ensuring adherence to screening and preventive care guidelines.
• Assist with phone triage, patient education, outreach, and conduct home visits as needed.
• Collaborate with a multidisciplinary team, including physicians, nurses, and support staff, under the supervision of the Center Medical Director.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties.
• Active, non-probationary state Nurse Practitioner license and DEA license.
• Legal US work authorization.

Preferred Skills

• Experience in primary care, geriatrics, or family medicine.
• Knowledge of HCC documentation, ICD-10 coding, and health assessments.
• Strong teamwork and cross-functional collaboration skills.
• Bilingual proficiency is a plus.
