Qualifications

• Master's or Doctorate in Nursing or NP certification in family, adult-gerontology, or gerontological nursing.
• Active, non-probationary NP license and DEA registration.
• US work authorization and certification in relevant specialties.
• Preferred experience in primary care, geriatrics, or internal medicine.
• Knowledge of HCC documentation, ICD-10 coding, and health risk assessments.
• Strong teamwork mindset and commitment to innovative population health models.
