Personal Touch Home Health Care Services Inc. is seeking a Nurse Practitioner in Youngstown, Ohio to provide high-quality primary care for adult Medicare patients, including routine and acute visits.

The role emphasizes wellness visits, care coordination, and strong patient relationships, with collaboration among physicians, nurses, and support staff. The position is full-time, approximately 40 hours per week, with a competitive salary based on experience and location and a broad benefits

Qualifications

• Master's or Doctorate in Nursing or NP certification (Family, Adult-Gerontology, or Gerontological) required/strongly preferred.
• Active, non-probationary NP license and DEA registration.
• US work authorization.
• Preferred: experience in primary care, understanding of HCC, ICD-10 coding, and population health.
• Team-oriented, innovative, and bilingual (preferred).
