Fairview Hospital / Cleveland Clinic is seeking a Nurse Practitioner, Advanced Practice Provider to deliver primary care to adult Medicare patients in a collaborative, population-health focused practice. You will perform routine and acute visits, wellness assessments, and care coordination with a multidisciplinary team.

Responsibilities: Deliver effective primary care to adult Medicare patients, including routine and acute visits, wellness assessments, and care coordination. Build strong patient relationships, educate patients on health conditions, participate in population health initiatives, and assist with phone triage and outreach. Conduct home visits when needed and collaborate with a multidisciplinary team focused on primary care, care coordination, and population health.
Qualifications: Master’s or Doctorate in Nursing, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioners, active state license, DEA license, and US work authorization. Preferred experience in primary care, understanding of HCC documentation, ICD-10 coding, and health risk assessments. Bilingual skills are a plus.
