Health, Inc in Rhode Island is seeking a Nurse Practitioner to deliver high-quality, value-based primary care for adult Medicare patients. The NP will work independently to conduct visits, wellness exams, and care coordination while collaborating with a multidisciplinary team.
You will participate in population health initiatives, provide patient education, and assist with home visits as needed. A DEA license, NP certification, and US work authorization are required.
The Nurse Practitioner provides effective, equitable, value-based primary care to adult Medicare patients, focusing on keeping them healthy and out of the hospital. They work independently to conduct office visits, wellness exams, and care coordination, building trust and meaningful relationships with patients. The role involves collaborating with a multidisciplinary team, participating in population health initiatives, educating patients, and conducting home visits as needed.
Full-time position, approximately 40 hours/week, with a competitive salary range.
