Qualifications

• Active NP or PA certification (ANCC/AANP or NCCPA).
• State licensure in WA or ability to obtain by start date.
• DEA licensure or prescriptive authority where required by state.
• Reliable transportation to patient homes.
• Ability to collaborate with interdisciplinary team.

Responsibilities

• Conduct comprehensive assessments (history, vitals, exam).
• Provide house-call care to patients.
• Maintain accurate documentation and ICD-10 coding.
• Collaborate with care team across settings.
• Educate patients and caregivers on care plans.
