Job summary

Optum Home & Community Care seeks a Part-time Nurse Practitioner or Physician Assistant for Spokane Valley, WA to provide home- and community-based care. This role offers a $30,000 sign-on bonus or $40,000 student loan repayment with no on-call, no weekends, and no holidays required.

As part of the Senior Community Care program, you will conduct assessments, coordinate care, and collaborate with a multidisciplinary team to improve patient outcomes in nursing homes and senior living settings.

Qualifications

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state with no disciplinary actions.
• Driver's license with reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.

Responsibilities

• Conduct comprehensive assessments including history, exam, vitals and documentation.
• Support members during a short term sub-acute rehabilitation episode.
• Communicate and collaborate with the interdisciplinary care team.
• Perform advanced illness and advanced care planning conversations.
• Counsel and educate patients and caregivers.

Education

ANCC or AANP Family/Adult/Geriatric Primary Care certification

NCCPA certification and state licensure
