Job summary

Optum Home & Community Care in Yakima, WA seeks a Nurse Practitioner or Physician Assistant to join its House Calls team. This field-based role delivers in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care coordination.

You will perform comprehensive exams, review histories, manage medications, and coordinate with primary care providers. Flexible schedules between 24 and 40 hours, no on-call, no weekends, no holidays, and opportunities

Qualifications

• Active ANCC or AANP certification for NP (Family, Adult, Geriatric, or Adult-Gerontology Primary Care).
• Current NCCPA certification and applicable state licensure for Physician Assistants.
• Active, unrestricted licensure in the state of practice and a valid driver's license.

Responsibilities

• Conduct in-home health assessments for health plan members.
• Review medical history, reconcile medications, obtain vitals, and perform exams.
• Complete evidence-based screenings and point-of-care testing as appropriate.
• Document diagnoses to support care planning and care management.
• Communicate assessment findings to primary care providers to close gaps in care.
• Identify urgent or emergent situations and intervene as needed.
• Educate patients about diseases, medications, and treatment plans.
• Assess social determinants of health and refer to community resources.
