Job summary

Optum Home & Community Care, part of Optum, is expanding its HouseCalls program in Yakima, WA. The role supports NP or PA professionals to perform in-home health assessments for Medicare Advantage members in a field-based setting with no prescribing required.

Responsibilities include history review, vitals, screenings, and collaboration with PCPs to close care gaps while educating patients and addressing social determinants of health.

Qualifications

• Active ANCC/AANP or NCCPA certification as required.
• Active, unrestricted state licensure.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and travel to patient homes.

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other plan members.
• Perform medical history reviews, vital signs, and physical exams during visits.
• Provide education to patients and coordinate with PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
