Qualifications

• LTCFS certification must be pursued/completed online screener training and become certified as a functional screener.

Responsibilities

• Schedule member appointments for annual or change-based functional eligibility re-determinations.
• Use the DHS screening tool to assess members’ functional abilities, health, and required supports.
• Collaborate with staff to ensure assessment data aligns with internal records.
• Conduct in-home, face-to-face interviews and enter assessment data into the designated system.
• Submit documentation promptly according to guidelines.
• Attend meetings and training to stay updated on processes and state directives.
• Participate in quality assurance activities to maintain screening competence and reliability.
• Maintain required certification through ongoing skills testing per Wisconsin standards.
