Job summary

Humana is seeking a Long-Term Care Functional Screen Specialist to support Wisconsin Family Care members in Eau Claire County. You will conduct in-home interviews, assess needs using the DHS tool, and determine care levels under state guidelines.

Qualified candidates have a Health/Human Services degree or RN license with experience, and LTCFS certification. Travel up to 50% within the region and a 40-hour work week are typical for this remote role.

Qualifications

• Four-year bachelor's degree in Health or Human Services field with one year of experience OR a Wisconsin licensed RN with one year of Family Care experience.
• LTCFS certification is required; must complete online screener training and become certified.
• Preferred: LTCFS certification; prior Family Care case management or screener experience.

Responsibilities

• Schedule member appointments for annual or change-based functional eligibility re-determinations.
• Use the DHS screening tool to assess members' functional abilities, health, and required supports.
• Collaborate with staff to ensure assessment data aligns with internal records.
• Conduct in-home, face-to-face interviews and enter assessment data into the designated system to determine level of care.
• Submit documentation promptly according to established guidelines.
• Attend meetings and training to stay updated on processes and state directives.
• Participate in quality assurance activities to maintain screening competence and reliability.
• Maintain required certification through ongoing skills testing per State of Wisconsin standards.
