Job summary

Molina Healthcare is seeking a healthcare support professional to conduct long-term care functional screen assessments across Wisconsin locations. The hybrid role requires local travel and monthly meetings at a Molina office, with LTCFS coordination for new enrollments, annual reviews, and condition changes.

The candidate will work with the ICT and family to complete assessments, ensure data accuracy, and maintain LTCFS certification.

Qualifications

• At least 1 year of health care experience with adults with disabilities or frailty.
• LTCFS certification.
• Bachelor's degree in social work/health-related field or active RN/LPN/LVN license where required.
• Valid driver's license and reliable transportation for state travel.
• Ability to support staff development and balance team and independent duties.
• Experience with data management, project coordination and training.
• Ability to explain complex material to groups and individuals.
• Proficiency with Microsoft Office suite.

Responsibilities

• Schedule member LTCFS assessments and home visits.
• Use the Wisconsin LTCFS to assess member abilities, health status, and supports.
• Complete LTCFS with member, family, and ICT.
• Coordinate with member ICT for enrollment and changes.
• Ensure data consistency between assessments and member records.
• Use spreadsheets to track LTCFS timeframes.
• Submit accurate documentation per guidelines.
• Attend trainings to maintain knowledge of LTCFS tool and processes.
• Participate in quality assurance activities and audits.
• Maintain LTCFS certification via CST.
