The LPN will administer medications, monitor vital signs, assist with ADLs, and maintain accurate documentation while adhering to infection control standards and facility policies.

Qualifications

• Valid, unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification as required by facility policy.
• Strong interpersonal and communication skills; ability to work with residents, families, and the care team.

Responsibilities

• Provide direct nursing care to residents under RN supervision or physician orders.
• Administer medications and treatments per orders and facility policy.
• Take and record vital signs and monitor changes in condition.
• Assist residents with ADLs and provide wound care as allowed.
• Document care provided and communicate resident status to the team.
• Adhere to infection control protocols and facility policies.
