Job summary

Ryders Health is seeking a compassionate Licensed Practical Nurse (LPN) to join Cheshire House in Waterbury, CT. This full-time role focuses on resident-centered care, direct nursing duties, and collaboration with an experienced healthcare team.

As an LPN, you will administer medications, monitor vitals, assist with ADLs, and document care in electronic records. A CT LPN license and CPR/BLS are required; prior long-term care experience is a plus.

Qualifications

• Active LPN license in Connecticut.
• Graduate of an accredited practical nursing program.
• CPR/BLS certification as required by facility policy.
• Strong interpersonal and communication skills.
• Compassionate, resident-centered approach with good organizational and time-management skills.
• Experience in long-term care or skilled nursing preferred.

Responsibilities

• Provide direct nursing care to residents under RN supervision or physician orders.
• Administer medications and treatments per orders and facility policy.
• Record residents’ vital signs and monitor for condition changes.
• Assist residents with ADLs such as bathing, dressing, feeding, mobility, hygiene.
• Perform basic wound care and dressing changes per policy.
• Collect specimens for lab tests as ordered.
• Observe and document residents’ status and report changes to RN/physician.
• Maintain accurate documentation of care and observations in medical records.
• Communicate with residents, families, and healthcare team about needs and concerns.
• Adhere to infection control protocols and facility standards.
