The LPN will administer medications, monitor residents, and collaborate with RNs and physicians to ensure high-quality care. A CT license and CPR/BLS are required.

Qualifications

• Must hold a valid CT LPN license.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification as required by facility policy.
• Strong interpersonal and communication skills.
• Experience in long-term care preferred.

Responsibilities

• Provide direct nursing care to residents under supervision or orders.
• Administer medications and treatments per orders and policy.
• Record vitals and monitor changes in condition.
• Assist with ADLs such as bathing, dressing, feeding, mobility.
• Perform wound care and ostomy care as allowed.
• Collect specimens for lab tests as ordered.
• Document care and observations in medical records.
• Communicate with residents and families about care needs.
• Adhere to infection control protocols and standards.
