Job summary

Ryders Health Management is seeking a compassionate Licensed Practical Nurse (LPN) to join Cheshire House in Waterbury, CT for a part‑time 16‑hour schedule (7a‑3p). You will deliver direct resident care under RN supervision, administer medications per orders, monitor vital signs, assist with ADLs, document care, and communicate with families as needed in a supportive team environment.

Qualifications

• Unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification as required by facility policy.
• Strong interpersonal and communication skills with residents and families.

Responsibilities

• Provide direct nursing care to residents under the supervision of an RN or physician orders.
• Administer medications and treatments per orders and facility policy.
• Take and record vital signs and monitor for changes in condition.
• Assist with activities of daily living (ADLs) as needed.
• Perform basic wound care and dressing changes when within scope.
• Collect specimens for labs as ordered.
• Document care provided and resident observations in medical records.
• Communicate with residents, families/POAs, and the healthcare team about status and needs.
