Job summary

Eventide on Eighth Care Center in Moorhead, MN is seeking an LPN to work the Part-time NOC shift (6:30 pm–7:00 am) with every third holiday rotation. The Charge Nurse will provide guidance and ensure care meets federal, state, and local standards.

The ideal candidate has an active MN LPN license or can obtain upon hire; LTC experience preferred. The role includes medication administration, care planning, accurate EMR documentation, and collaboration with physicians and care staff.

Qualifications

• Active MN LPN license or ability to obtain upon hire.
• Experience in long-term care preferred.
• Assesses residents’ needs via physical assessments, observations, and interviews.
• Delivers direct nursing care according to policies and standards.
• Documents care in residents’ electronic medical records daily.
• Maintains communication with care teams and families.

Responsibilities

• Provide direct nursing care to residents on assigned shifts.
• Administer medications and monitor effectiveness.
• Observe and update plans of care based on physician orders.
• Ensure accurate documentation in EMR and reporting to Charge Nurse.
• Coordinate resident care conferences and communicate with families.
