Qualifications

• Valid, unrestricted LPN license in CT.
• Graduate of an accredited practical nursing program.
• Current CPR/BLS certification as required by facility policy.
• Strong interpersonal and communication skills.
• Compassionate, resident-centered approach with good time-management.

Responsibilities

• Provide direct nursing care under RN supervision or physician orders.
• Administer medications and treatments per orders and policy.
• Take and record vital signs and monitor changes in condition.
• Assist with ADLs such as bathing, dressing, feeding, mobility.
• Perform basic wound care and dressing changes where allowed.
• Collect specimens for lab tests as ordered.
• Observe and document status; report changes to supervising staff.
• Maintain timely documentation in medical records.
• Communicate with residents, families and healthcare team.
