Job summary

Baystate Health is seeking a part-time Licensed Practical Nurse to join Baystate Hospice in Springfield, MA. The role coordinates hospice services, triages patient needs, and supports clinical management.

Shifts are three 8-hour days (7:30am–4:00pm) with weekends rotating. Responsibilities include EMR documentation, scheduling, care transitions, and collaboration with physicians, families, and staff to ensure safe, cost-effective care.

Qualifications

• 3 years hospice/home health or similar experience.
• BLS certification (American Heart Association)
• LPN licensure required (state board)

Responsibilities

• Coordinate hospice services and triage patient needs.
• Enter/edit orders and manage documentation in EMR.
• Facilitate transitions of care between hospitals, home, and facilities.
• Collaborate with physicians, families, and staff to ensure quality care.
• Assist with scheduling, billing information, and resource utilization.
