Baystate Health is seeking a Licensed Practical Nurse (LPN) Patient Care Coordinator for hospice care in the Springfield area. This non-hands-on role focuses on coordinating discharge planning and arranging home-based hospice services, working with hospital staff, hospice teams, and families.

The LPN will facilitate transitions from hospital to home, coordinate equipment and services, and ensure timely, seamless discharge while maintaining compliance with policies and scope-of-practice

Competenze

• LPN license in Massachusetts is required.
• Drivers license required.
• BLS certification required by American Heart Association.

Mansioni

• Coordinate transitions from hospital to home for hospice patients and manage discharge planning.
• Collaborate with hospital discharge planners, nurses, physicians and families to ensure continuity of care.
• Coordinate home hospice services, equipment, supplies and resources after discharge.
• Communicate discharge plans and next steps to patients and families.
• Identify and resolve barriers to discharge and coordinate community resources.
