Job summary

Baystate Health is seeking a Licensed Practical Nurse (LPN) Patient Care Coordinator to join our Hospice Team. This role coordinates hospital-to-home discharges, collaborates with discharge planners, RNs, physicians, hospice providers, and families to ensure timely, safe transitions and coordinated home-based services.

Based in the Home Health Office in West Springfield, this part-time 32-hour position focuses on coordination rather than direct patient care, helping remove barriers to discharge

Qualifications

• LPN License in Massachusetts is required.
• Valid Driver's License is required.
• BLS Certification from American Heart Association is required.

Responsibilities

• Coordinate transitions from hospital to home for hospice patients, ensuring a safe, timely, and seamless discharge process.
• Collaborate with hospital discharge planners, registered nurses, physicians, hospice providers, and community partners to facilitate continuity of care.
• Coordinate home hospice services, medical equipment, supplies, and other resources needed to support patients upon discharge.
• Communicate with patients and families regarding discharge plans, available services, and next steps in the transition home.
• Identify and help resolve barriers to discharge, including service availability, equipment needs, and coordination of community resources.
• Facilitate the exchange of patient information and discharge documentation among hospital staff, hospice teams, and receiving providers.
• Track transition progress, follow up on outstanding needs, and help ensure required services are arranged before the patient returns home.
• Maintain accurate documentation and support compliance with organizational policies, hospice procedures, and applicable LPN scope-of-practice requirements.
