Job summary

Mass Digital Health in Quincy, MA is looking for a part-time Licensed Practical Nurse to assist in coordinating patient care. The role includes handling patient inquiries, scheduling visits, and collaborating with interdisciplinary teams.

The ideal candidate will have a current LPN licensure in New York and preferably a year of nursing experience. This position offers a schedule of 8 a.m. to 4 p.m. every Saturday and Sunday with holiday rotation.

Qualifications

• Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
• 1 year of nursing practice as a Licensed Practical Nurse preferred.

Responsibilities

• Assists in coordination and implementation of the patient plan of care.
• Handles phone calls from the patient, primary caregiver, or designee regarding any questions or concerns.
• Collaborates with interdisciplinary teams, RNs, or Team Manager.
• Coordinates and schedules patient visits.
• Provides timely documentation/data entry per Hospice time frames and deadlines.
• Communicates current health status and needs of patients to appropriate team professionals.
• Assists the team and Team Manager with other administrative functions, as necessary.
