Baystate Health is seeking a Licensed Practical Nurse (LPN) Patient Care Coordinator for hospice care in the Springfield area. This non-hands-on role focuses on coordinating discharge planning and arranging home-based hospice services, working with hospital staff, hospice teams, and families.

The LPN will facilitate transitions from hospital to home, coordinate equipment and services, and ensure timely, seamless discharge while maintaining compliance with policies and scope-of-practice

Qualifications

• LPN license in Massachusetts is required.
• Drivers license required.
• BLS certification required by American Heart Association.

Responsibilities

• Coordinate transitions from hospital to home for hospice patients and manage discharge planning.
• Collaborate with hospital discharge planners, nurses, physicians and families to ensure continuity of care.

Our team provides compassionate, patient-centered end-of-life care across hospital, home, and community settings. Working closely with hospital discharge planners, registered nurses, physicians, hospice providers, and families, the LPN Care Coordinator facilitates the transition from hospital to home by coordinating services, communicating care needs, and ensuring necessary resources are in place for a safe and seamless discharge. This is a non-hands-on patient care role focused on coordination and continuity of services.The LPN Care Coordinator plays a meaningful role in supporting patients and families during the transition home by facilitating discharge planning, coordinating home-based services and equipment, and addressing barriers to care.
