The position offers a 20-hour workweek with compensation ranging from $45,000 to $50,000 per year, reflecting leadership responsibilities in a home health setting. Prior community/home health experience is required.

Qualifikationen

• Active RN license valid in NJ.
• BSN or related health degree from an accredited program.
• Minimum 2 years RN experience in community health or home health.
• Valid driver's license and active auto insurance.
• Strong English communication and physical ability for direct care.

Aufgaben

• Evaluate and document clients’ biophysical, psychosocial, and educational needs. Deliver preventative, supportive, and restorative care per standards.
• Administer and document prescribed medications and perform clinical treatments per Plan of Care.
• Coordinate scheduling for multidisciplinary appointments and therapies; maintain productivity.
• Interpret health status implications; report changes to Director of Nursing and physician.
• Adhere to infection control, HIPAA, and safety standards for clients and staff.
• Maintain complete client records and timecards; contribute to quality improvement.
• Reports to Director of Nursing.
