Shineforth is seeking a Part Time Intensive Care Coordinator in Farmville, VA to support youth and families in the community. You will help create teams, set goals, plan transitions, and monitor progress to keep youth safely in their home and community.

You will participate in weekly supervision, attend required trainings, and implement the 10 principles and 4 phases of High Fidelity Wraparound with fidelity, while coordinating natural supports and community linkages.

Formación

• Bachelor’s degree required with at least 2 years of direct clinical experience providing children’s mental health services.
• Completion of “High Fidelity Wraparound” training preferred.

Responsabilidades

• Work with youth and families to create teams, develop goals, plan transitions, and monitor progress.
• Attend mandatory county and state trainings on ICC and High Fidelity Wraparound.
• Implement the 4 phases of High Fidelity Wraparound: engagement, planning, implementation, transition.
• Ensure the principles and phases of the High Fidelity Wraparound process are delivered with fidelity.
• Seek coaching, supervision, and professional input when needed; participate in weekly supervision.
• Work with families to develop natural supports in their communities.
• Facilitate all youth and family team meetings; ensure active participation.
• Ensure youth and family voice and choice is present at all meetings.
• Identify and link youth and families to natural and community resources.
• Write, revise, and distribute team plans of care and FAPT reports as needed.
• Conduct initial and discharge assessments to capture strengths and needs of youth and family.
• Provide written and verbal reports to the FAPT for ongoing monitoring.

Educación
