Job summary

Shineforth Treatment Foster Care in Virginia Beach is seeking a PRN Intensive Care Coordinator to support youth and families in the community. You will build teams, set goals, plan transitions, and monitor progress to help youths remain with their families in the community.

You will attend county and state trainings on ICC and High Fidelity Wraparound, implement the 4 phases, facilitate team meetings, and prepare team plans and reports.

Qualifications

• Bachelor’s degree with at least 2 years of direct clinical experience providing children’s mental health services.
• Completion of High Fidelity Wraparound training preferred.

Responsibilities

• Work with youth and families to create teams, develop goals, plan and coordinate transitions, and monitor progress to keep youth in the community with their family.
• Attend mandatory county and state trainings on ICC and High Fidelity Wraparound.
• Implement the 4 phases of High Fidelity Wraparound: engagement, planning, implementation, transition.
• Facilitate all youth and family team meetings with full participation of all members.
• Write, revise, and distribute team plans of care and FAPT reports as needed.
• Conduct initial and discharge assessments including strengths and needs of youth and family.
• Provide written and verbal reports to the FAPT for ongoing monitoring.

Our youth and families are supported by a collaborative team of specially trained clinical staff and foster parents, who provide daily behavioral health interventions that align with each child’s individualized treatment plan.**What we are looking for:**An Intensive Care Coordinator to provide services to youth and families in the community. Ensure all team members are present and participate.* Ensure youth and family voice and choice is present at all meetings.* Identify and link youth and families to natural and community resources.* Write, revise, and distribute team plans of care for each team meeting and FAPT reports as needed.* Conduct an initial and discharge assessments to include strengths and needs assessments of the youth, family, extended family and the community.* Provide written and verbal reports (as required) to the FAPT for ongoing monitoring.**What you will need:**Bachelor’s degree with at least 2 years of direct clinical experience providing children’s mental health services to children with a mental health diagnosis.
