Job summary

Youth Villages seeks a High Fidelity Wraparound Facilitator to support youth and families in North Carolina. You will manage a small caseload, coordinate team-based services, and develop care plans that promote safety and stability at home and school.

Responsibilities include facilitating wraparound, on-call crisis support, documenting in EMR, and participating in regular supervision and trainings to strengthen clinical skills and outcomes for youth ages 3 to 17.

Qualifications

• Requires a clinical degree and licensure in NC
• Experience with Wraparound or similar care coordination preferred
• Ability to work with youth and families in community settings
• Must have reliable transportation and valid driver's license

Responsibilities

• Facilitate High Fidelity Wraparound services with a strengths-based approach
• Develop and monitor Wraparound Plans with families and partners
• Provide on-call crisis support as needed
• Attend weekly meetings for professional development and clinical consultation
• Maintain accurate EMR documentation and timely reporting
• Support youth to remain in home, school, and community settings
