Qualifications

Responsibilities

• Conduct in-person behavioral health screening at designated clinic days.
• Complete clinical assessments and provide brief individual therapy.
• Collaborate with primary care providers, behavioral health clinicians, and partner-site staff.
• Maintain a small Core/Tier caseload, with clinic growth.
• Provide virtual therapy when appropriate.
• Maintain timely clinical documentation and coordinate patient referrals.
