InterCare Community Health Network in Holland, MI, seeks a part-time Behavioral Health Consultant (20+ hours/week) to support behavioral health integration in primary care within a Federally Qualified Health Center setting.

The role includes consultation to providers, brief screenings, short-term interventions, and crisis management, with referrals and team-based care. Masters in Social Work with LLMSW/LMSW preferred; LPC considered; strong commitment to access to care.

Qualificações

• Master's degree in Social Work; LLMSW/LMSW licensure preferred.
• LPC consideration may be given based on experience.
• Experience in integrated primary care and behavioral health.
• Ability to conduct brief screenings and referrals.

Responsabilidades

• Provide consultation to medical providers on behavioral health in primary care.
• Conduct brief screenings for adults, adolescents, and children for behavioral health intervention.
• Provide short-term behavioral interventions and crisis management.
• Participate in ongoing training and provide consultation to clinic staff on mental health topics.
• Function within an integrated primary care team model.
