Job summary

21st Century Home Health Services in San Francisco is seeking a Speech Therapist/Language Pathologist to join a growing home health team serving patients in SF and surrounding counties.

You will work in partnership with case managers to evaluate and treat communication, swallowing, and cognitive disorders in the home, with autonomy, flexible scheduling, and a comprehensive benefits package.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and behavior as it relates to illness/injury; culture in health care.
• Home health care experience preferred.
• High acuity patient experience is a plus.
• Interdisciplinary team experience is highly desired.
• Case Management fundamentals desired.

Responsibilities

• Provide skilled speech-language pathology services to patients in home settings.
• Evaluate and treat speech, language, cognitive and fluency disorders.
• Serve as educator, patient advocate, and member of the care team.
